The forms below are to be completed by the principal investigators (PI) and submitted to the Institutional Animal Care and Use Committee (IACUC) via email to: IACUC@uml.edu for review and approval of all activities and procedures that apply to all live vertebrate animals used for research, testing, and teaching. The forms are in Microsoft Word docx format.

Animal Occupational Health Program

The purpose of the Animal Occupational Health Program (AOHP) is to identify, evaluate, manage, and reduce potential health risks associated with the care and use of animals. This program is a collaborative responsibility of Office of Research Integrity (ORI), the Environmental Health & Safety (EHS), and Lowell General Hospital (LGH) Occupational Medicine.

All individuals (faculty, postdoc, staff, researcher, student, non-UMass Lowell (UML) sponsor, collaborator, and visiting scientist) working with animals or listed on an IACUC protocol or working in a facility where animals are housed must enroll in the Animal Occupational Health Program. This includes:

  • Anyone listed on an IACUC protocol as research personnel
  • Anyone working with animals for research, testing or teaching purposes
  • Anyone having contact or exposure to laboratory animals (including casual exposure)
  • Anyone entering the animal facilities

Enrollment consists of annual submission of the AOHQ form. Everyone involved with laboratory animals must submit a new AOHQ form:

  • Before being added to or listed on an IACUC protocol
  • Before first contact with laboratory animals
  • When there’s a change in health status (e.g., pregnancy, diagnosis of an immune disorder etc.)
  • Whenever exposure information changes (e.g., using new animal model)
  • At least once per year

Animal Occupational Health Questionnaire forms missing information will be returned to the individual.

Note: It is recommended that the Applicant has had a tetanus shot within the last 10 years and a physical within the last 5 years.
