To apply for a STEM Extension I-20, students must upload the following documents to the STEM Extension Request e-form:

  • Copy of passport ID page (note: Foreign passports should always be valid for at least 6 months in the future)
  • Most recent copy of Form I-94
  • Copy of Post-OPT EAD
  • Unofficial copy of UMass Lowell transcript (printed within the last 2-4 weeks)
  • STEM Reporting Deadline Calculator (xls)
  • Completed and signed in inkForm I-983*
