How to access the UMass Lowell Student Health Portal and upload health records

Health Services Patient Portal

Please submit all documentation through the Student Health Portal. For electronic medical record security, we are unable to accept forms by fax or email.

Per Massachusetts regulations, as a condition of enrollment at the university and subsequently, treatment at Health Services, all full-time students, and part-time health majors are required to have their health forms on file.

ONLINE STUDENTS: Exclusively online students (i.e. those without required on-campus symposia, intensives, seminars, etc.) are not required to submit health history, TB risk and immunization records if they do not participate in any activities or instruction on campus, regardless of major. Students who participate in any type of on-campus course requirements must submit health and immunization records for review and verification. Students in certain degree programs with clinical/community placements may be required to comply with host agency immunization requirements. Students who transition to any on-campus courses during their career at UMass Lowell will be required to submit immunizations and health history forms per policy.

Online Health Record

All newly enrolled students must complete the following mandatory electronic health forms located in the Forms section of their Student Health Portal (link above):

The PDF files are included above as a preview, but forms must be completed electronically. 
In addition, please download the Immunizations and Physical Examination forms below and bring them to your medical provider for completion and signature. Once signed by your provider, please upload a copy to the Patient Portal. Alternatively, we will accept a medical provider's standard form as long as it includes your complete immunization record and date of your last physical.
  • Immunizations (pdf): Upload medical documentation signed by medical provider.
    1. Two doses MMR (Measles, Mumps, Rubella) vaccine or a positive MMR IgG blood test (laboratory test documentation required).
    2. Two doses of Varicella vaccine for Chicken Pox or documented history of disease, or positive blood varicella IgG blood test (laboratory documentation required).
    3. Three doses Hepatitis B (Engerix B) or two doses Hepatitis B (Heplisav), or positive Hepatitis B surface antibody blood test (laboratory documentation required).
    4. One dose of Tdap administered on or after your 11th birthday is required. Additional doses (boosters) of Tdap or Td are not required, but highly recommended every 10 years.
    5. Meningococcal vaccine: All newly enrolled full-time students 21 years of age or younger must receive a Meningococcal ACWY vaccine on or after their 16th birthday; or, sign the Department of Public Health's Meningococcal Disease and Vaccination Waiver Form (pdf).
    6. COVID-19 vaccine: No longer required, however, students are strongly recommended to receive this vaccine and remain up-to-date on recommended boosters.
  • Physical Examination Form (pdf): Physical examination should be performed within the last two years.

Due Dates

All health records are required by Aug. 1 for fall enrollment and Jan. 1 for spring enrollment.

Failure to submit mandatory health records will result in a hold on your registration and possible removal from enrollment.
