Mandatory Medical Records Requirements
Per Massachusetts law, as a condition of enrollment at the university and subsequently, treatment at Health Services, all full-time students, and part-time Health majors are required to have the following health forms on file.
Online Health Record
Students must complete all health forms online and upload supporting medical documentation through the Health Services Patient Portal link on the Health Services website.
Students must create an online portal account prior to submitting documents. You must use your name as it appears in SIS.
No paper copies or faxes are accepted.
The following must be completed by the student:
- Health History
- Students must complete both parts of the Health History form.
- Tuberculosis Risk Assessment Questionnaire
- View list of TB-Endemic countries and answer questions.
- Immunizations
- Upload medical documentation of required immunizations signed by medical provider.
- Physical Examination
- Download the Physical Examination Form and bring to your medical provider for completion; upload to the Patient Portal. We will also accept the medical provider's form. Physical examination should be performed within the past 12 months.
Due Dates: All health records are required by August 1 for fall enrollment and January 1 for spring enrollment. Records are reviewed in the order in which they are received. During periods of high number of submissions please be advised that it may take 3-7 business days for records to be verified and reviewed.
Note: Failure to submit mandatory health records will result in a hold on your registration that will impact your ability to register for upcoming semesters.
