Please download and complete these required health and liability forms. NOTE: These forms must be submitted before June 26th @ 6pm (which is also the date/time of parent orientation). You can fill out these forms at the Campus Recreation Center but we recommend that parents download/print them out now & send the completed forms to the attention of Meg Flynn, UML Campus Rec Center, 100 Pawtucket St. Lowell, MA 01854; you may also fax the forms to us at 978-934-3041.

Camp is a tuition-for-service program based on confirmation enrollments and secured deposits. A $50 per-session non-refundable non-transferable deposit is required. Enrollment forms will be processed as outlined in the camp brochure. I understand that the reservation will be held by my deposit until May 31st, 2013. At that time, the balance in full must be received. If full payment is not received by this time, my reservation(s) will be cancelled.
